Supplemental Form not Available for Customers

Supplemental Forms must remain within their due date for customers to complete them after registration. If a form is no longer available, the Due Date may have already passed.

To change the Due Date, please use the following steps:
  1. Click Home tab
  2. Click Setup beneath desired Season
  3. Select Registration Forms from right sidebar
  4. Click Edit next to desired Supplemental Form
  5. Set Due Date to a later date
  6. Click Save and Continue
