Supplemental Forms should be available for customers to complete after registration.  If the form is not available, the Due Date for the Supplemental Form may have passed:

To change the Due Date, please use the following steps:
  1. Click Home tab
  2. Click Setup beneath desired Season
  3. Select Registration Forms from right sidebar
  4. Click Edit next to desired Supplemental Form
  5. (Optional) Change Due Date dropdown box to Specific Date
  6. Set Due Date to a later date
  7. Save and Continue
