Adding Parent/Guardian Questions

If the participant is a child under 18 years, these primary parent/guardian questions are required on the basic registration form:
  • name
  • email
You can add the following optional questions:
  • address
  • date of birth
  • phone numbers
  • gender
To add parent/guardian questions:
  1. On Home tab, click Setup beneath appropriate Season 
  2. Click Registration Forms on right sidebar
  3. Click Customize registration form
  4. In Parent/Guardian section, click respective Add button applicable question
  5. Click Save and continue
Note:  If participant is an adult (over 18 years), the parent/guardian questions do not appear on form
