If the participant is a child under 18 years, these primary parent/guardian questions are required on the basic registration form:
- name
You can add the following optional questions:
- address
- date of birth
- phone numbers
- gender
To add parent/guardian questions:
- On Home tab, click Setup beneath appropriate Season
- Click Registration Forms on right sidebar
- Click Customize registration form
- In Parent/Guardian section, click respective Add button applicable question
- Click Save and continue
Note: If participant is an adult (over 18 years), the parent/guardian questions do not appear on form
