If the participant is a child under 18 years old, the following primary parent/guardian questions are required on the basic registration form: Name, Email, and Date of birth. You can also add optional questions such as Address, Phone numbers, and Gender.
- On Home tab, click Setup beneath appropriate Season
- Click Registration Forms on right sidebar
- Click Customize registration form
- In Parent/Guardian questions section, click respective Add button for applicable question
- Click Add form element dropdown and select Custom question to add more custom questions
- Click Save and continue
Note: If participant is an adult (over 18 years), the parent/guardian questions do not appear on the form
