The following types of questions can be added for a secondary parent/guardian:

  • name
  • email
  • address
  • phone numbers
  • date of birth
  • gender

To add Secondary parent/guardian questions:
  1. Go to SETUP below the season name
  2. From SETTINGS select Registration forms , click Customize registration form
  3. At the bottom part of the page select "Add" next to Secondary parent/guardian
  4. Save and Continue
