In Camp and Class Manager there can only be one registration form per season. Once custom questions have been created they can be set to display only to registrants from certain sessions. Use this article: Creating Custom Questions for instructions on Creating Custom Questions.

- Select Setup beneath season name
- Select Settings on right-hand side of page
- Select Registration forms
- Select Customize Registration Forms
- Select Add Form Element
- Check questions to include
- Select Apply
- On Registration Form scroll down to Custom Question
- Select Edit
- Select Session applicability tab
- Select/deselect applicable Sessions and Apply
- Repeat steps 5-11 for each question
