To add your own form questions to a volunteer event follow the steps below:

Note: Standard form questions (name, gender, date of birth) cannot be changed, modified, or removed
  1. Log in to ACTIVEWorks Endurance 
  2. Click VOLUNTEER
  3. Select desired event
  4. Under Actions, select Edit event
  5. Select Form questions
  6. Click edit form
  7. Select type of question to add (left navigation)
  8. Fill out required fields
  9. Click Save
  10. Click Save & continue
