To add your own form questions to a volunteer event follow the steps below:
Note: Standard form questions (name, gender, date of birth) cannot be changed, modified, or removed
- Log in to ACTIVEWorks Endurance
- Click VOLUNTEER
- Select desired event
- Under Actions, select Edit event
- Select Form questions
- Click edit form
- Select type of question to add (left navigation)
- Fill out required fields
- Click Save
- Click Save & continue
