Below are the instructions to add custom form questions:

Open Your Event after Logging ACTIVEWorks Endurance
Select Setup > Step 6: Form questions
Click Edit form
Select Custom questions

Select to create a desired question format

Type in Question label accordingly
- Note: Question label is what will be displayed to the public
Set the desired custom form question

Under Options, this is where you can adjust the availability settings (e.g whether you would like to have to require a response for this certain question)
You may also add this question to Form Question for Someone Else,so that it will appear on all of your registration forms.
You may also Adjust Advanced Settings in Form Questions

Click Apply
Click Save to return to Step 6: Form questions

Note: To make a change to existing questions, click pencil icon to edit
