Create Custom Report for Form Question Answers

Custom questions report can be generated for member responses of custom form questions. The report can be generated at package level by following steps below:

1. Log in Membership Manager at https://membership.active.com
2. Click Reports tab
3. Select Custom questions under Request report
4. Under Select filters, select Membership Status, Program, Package to generated report for
5. Select custom questions

• Select Choose custom questions to include only specific questions iin the report
• Select View all custom questions to include all custom questions

6. Click Request report
7. Click Refresh
8. Select Download under Actions once available to export 

Note: The following fields are included along side with selected custom questions in the report:

• Agency Name
• Program
• Package
• Package Option
• Membership Number
• Membership Status
• First Name
• Last Name
• Primary Phone
• Email Address
• Mailing Address
• Mailing Address 2
• Mailing City
• Mailing State
• Mailing Zip
• Mailing Country
