Add Custom Form Question

Below are the instructions to add custom form questions:

1. Open Your Event after Logging ACTIVEWorks Endurance
2. Select Setup > Step 6: Form questions
3. Click Edit form
4. Select Custom questions

5. Select to create a desired question format

6. Type in Question label accordingly
   • Note: Question label is what will be displayed to the public
7. Set the desired custom form question

• Under Options, this is where you can adjust the availability settings (e.g whether you would like to have to require a response for this certain question)
• You may also add this question to Form Question for Someone Else,so that it will appear on all of your registration forms.
• You may also Adjust Advanced Settings in Form Questions

6. Click Apply
7. Click Save to return to Step 6: Form questions

Note: To make a change to existing questions, click pencil icon to edit
