  • Vendor/ Seller must read "Global Payments, Inc. D/B/A: Active Network, LLC"
  • Address for vendor: "PO Box 744932, Atlanta, GA, 30384-4932"
  • Form must be signed and dated
  • TECs can be emailed to hyteksales@active.com
