If you would like to request a duplicate registration confirmation for your USA Hockey registration, follow these steps:
1. Go to www.usahockeyregistration.com
2. Click Request Duplicate Registration Confirmation in the Member Options section
3. Select Season, enter your First Name, Last Name, Birth Date and Zip Code
4. Click Submit
