Customers may send their supplemental forms to Organization either via email or in person.
Organization can manually upload the documents internally by following the steps below :


To Upload a supplemental form document internally :
  1. From Homepage click on the season name
  2. Click on Supplemental forms tab
  3. Beside the participant name click on Actions under Form column and select Manage uploaded files

  1. Click Upload more files and search and select the file
  2. Click on Save
Note: By default, when a form is uploaded, the Status will automatically be marked as Received.
