Customers may send their supplemental forms to Organization either via email or in person.
Organization can manually upload the documents internally by following the steps below :

To Upload a supplemental form document internally :
- From Homepage click on the season name
- Click on Supplemental forms tab
- Beside the participant name click on Actions under Form column and select Manage uploaded files
- Click Upload more files and search and select the file
- Click on Save
Note: By default, when a form is uploaded, the Status will automatically be marked as Received.
